Consent Statement

I hereby acknowledge that I have been informed by Aplus Hospital Hospitality Services Inc. (“Company”) via the “Information Notice on the Acquisition and Processing of Personal Data” (“Information Notice”) prepared in accordance with the Turkish Personal Data Protection Law No. 6698 (“KVKK”).

Except for the processing and sharing of personal data as outlined in the Information Notice, solely as necessary to fulfill contractual obligations, protect public health, and administer treatment and care services, I hereby give my explicit consent for the processing and storage of my personal data, in accordance with KVKK articles 5, 6, and 8 and for the purposes and legal grounds stated in the Information Notice, specifically to ensure the full and proper performance of the Company’s services. This includes:

• Service-related notifications, direct calls, surveys, and research activities through the mobile app and web portal;
• Application of programs related to Aplus, Acıbadem Healthcare Group, and/or affiliated partners;
• Verification of identity;
• Public health protection, treatment, care services, and scientific research activities;
• Compliance with legal and regulatory requirements;
• Verification of relationships with hospitals, medical centers, and affiliated institutions or consensus on services rendered by these organizations;
• Sharing information with the Ministry of Health and relevant public institutions when requested by law;
• Implementing necessary technical and administrative measures for data security within the systems and applications of hospitals and medical centers;
• Storage of data to improve and enhance the provided services;
• Compliance with requests and inspections by regulatory and supervisory authorities;
• Preservation of data as required by relevant legislation, including, but not limited to, medical diagnosis, healthcare services, planning and management of healthcare financing, increasing satisfaction, and conducting research;
• Communication through phone, SMS, email, and other means to inform about Company activities and services, reminders, new medical developments, and updates on new services that may be offered by the Company;
• Conducting special advertising, sales, marketing, profiling, statistical studies, surveys, and all types of electronic communications with similar purposes.

I grant my explicit consent under KVKK for the processing, storage, and sharing with third parties, of my personal data, including special category personal data, health data, and any personal data of my newborn child for whom I have parental authority, in line with the specified procedures and principles. I acknowledge that I have reviewed the information provided, am aware of my rights under the law, and understand that I can withdraw my consent at any time. I accept and declare that I give my explicit consent for the sharing of my personal and special category personal data, primarily with Acıbadem Healthcare Group and other third-party institutions as indicated above, for the stated purposes.
